If you were uncomfortable about crossing the Thai Malaysia border you could try AirAsia for a cheap flight from Thailand to Malaysia. They've got a special offer on at the moment for flights between Bangkok and Kuala Lumpar (I got a flight for 9.99 MR + taxes), but I think they also fly between Phuket and KL if that was more convenient.

Are there any safe border crossings into Malaysia at the moment or is it best to completely avoid?

I don't know about the security situation at present. When we were there (April '07)
there had been some problems near the border.
I had registered with the US Embassy in Bangkok and they sent me an email suggesting
I change my travel plans and not go thru S. Thailand by train. Since I had my wife and
13-year-old Granddaughter with me, I did change our plans and we flew Lufthansa from
KL to Bangkok and return.
Check with your Embassy in KL and/or Bangkok.

No worries. You can cross the border at Padang Besar by train or at Sadao by bus. These 2 borders in Songkla province are safe enough. Just avoid 3 other checkpoints on the east side (Sungai Kolok, Tak Bai & Buketa) which is situated in Narathiwat province. Its the best to avoid these 3 provinces (Yala, Pattani & Narathiwat). You can see many tourists travelling by train at the train station everymoring at Padang Besar immigration.;)
