I would like to get certified to dive in Koh Tao this October. Do you need to make advanced reservations, or can you find a place when you get there and make them a day or two in advance?

I got AOWD/Deep certification on Koh Tao 2 years ago (May 2005). I didn't made any reservation (nor in diveshop, nor in hotel) - it is easy to find accommodation and dive shop on arrival. Koh Tao is a quite small island with plenty options of accommodation / certification.
But October is not a best time for visiting Koh Tao. I was told that most of dive shops on Koh Tao are closed in October due bad weather.

Sorry to just jump into this thread...Does anyone know where the best place to dive would be around oct - nov in Thailand?

I think that October is not a good time for diving in Thailand :)
"Scuba season" Nov-Apr for the West coast (Andaman sea) and Apr-Aug for the Gulf of Thailand. So, September-October are "dead" for scuba diving.
But you can still try drift diving and wreck diving near Pattaya :)

The Diving Season

You can dive Thailand all year round. The best conditions exist from November to April, with whale sharks visiting from February to May, and manta rays all year round.

Late May to October brings monsoon winds and surface swells to the Andaman Sea, making some remote sites, such as Richelieu Rock and Hin Daeng, inaccessible and reducing visibility in general by about 30% at the open dive sites. October can also see heavy rains in the Andaman Sea.

The Phuket liveaboard season runs from early October to mid May, but there are a limited number of liveaboards to Similan Islands and Phi Phi Islands all year round.

For the Gulf of Thailand, the best months are May to August, when the Andaman Sea has it's low season.
