Partly depends on whether you are a traveller or actually living in the country/city. Most of the time, the capital city (or at least the biggest city) in the country is the most expensive one.
Just about a month ago, a survey's conclusion was that Moscow is the most expensive city in the world to live in, followed by London, Seoul and Tokyo.
This is based on around 200 items, such as housing, transport, food, entertainment etc. etc. There were only 143 cities which were surveyed, but I guess it gives a good idea.
Cheapest one was Asuncion in Paraguay. Other expensive cities were mainly European, with Copenhagen, Milan, Oslo, Zurich and Geneva all in the top 11.

For travellers on the other hand, you can travel cheap in Japan or Switzerland, and travel expensive in Paraguay, Bolivia or Syria if you want. One of the most expensive countries for travellers would surely be Iceland, as even a budget room in a hostel will set you back at least 80 dollars, and even a bed in a dorm is no less than 25 dollar, but that means you have to be lucky and travelling out of highseason.

And there are surprises as well of course. You should be surprised how expensive East Africa can be (even if on a budget) comparing to the local standard of living. But camping in Sweden (which is expensive) for example is much cheaper than it is in for example Italy or France.
