I heard of a $100 entry fee into Chile for US Citizens. Is this only at the airport? Do you pay it to customs or is it included in air fare? Does this entry fee also apply when crossing border from argentina to chile by bus?

It is only for people flying into the country - the fee is not paid crossing the border on land. The fee is not included in your tickets, and must be paid upon landing. You will get a stamp and ticket in your passport, which is good for as long as you maintain that passport (thus if you leave and come back into Chile, you won't need to pay again).

You can pay with Chilean money, U.S.A. dollars or travellers cheques. They don't take credit cards (or at least didn't when I was there). No other currency is accepted, and there is no means to get or change money prior to paying.

I seem to remember something similar in BA, only I think it was before leaving, they would not let you through to departures or something like that, without paying $18
