I am planning a 2-month trip, flying into Bangkok (from the U.S) and stay in Chiang Mai for about a week, then fly to Vietnam for three weeks and return to Bangkok to explore South Thailand for the next 2-3 weeks. I know that I will be granted a 30-day Exemption on my first entry, but do I need to apply for a visa or an extension for my second entry into Thailand? I reeeallly don't want to pay $200 for a multiple-entry visa.

This suggests you'll get another entry visa for your 2nd entry without issue.
That is provided that you don't stay longer than 30 days on your first visa.
