That is what everyone here has said...
There are places where the risk is lower and taking a gamble could be justified to some degree, but the places the OP mentioned aren't lower risk.

You didn't specify the time of year you plan to plan to be in the San Blas Islands and Roatan. Mosquitos will be more plentiful during the wet season. If you live in the U.S., know that Malarone is available in generic form as atovaquone/proguanil. My prescription drug plan allows me to purchase a 90-day supply for US$25. I've had no side effects using this antimalarial for as long as 90 days. As previously mentioned by others, talk to your doctor; and consult the CDC's Web site.

I traveled in Nicaragua, Honduras (including Guanaja Island), El Salvador, Guatemala and Belize from June 24 to Aug. 8, 2018. While my travel companion and I decided not to use an antimalarial (after talking with our guide, who lives in Honduras), we did cover up at dawn and dusk; and in shaded areas. My friend used insect repellent; I did not.

My parents did a trip to Ethiopia back in the late 1990's and my mother did the Malaria medication thing before and during and after the trip and STILL got it and ended up in the hospital for 5 days with it. Amazingly enough, it took 5 months after the trip before the Malaria hit. Today, she's doing wonderful at 84 years old. Mind you, she has not been back to a malaria country since. It really is up to the person. My Dad never has a problem with bugs where my Mother does. I inherited my mother's love for bugs.
