i would highly recommend against a rental car. unless you are very familiar with central america ie. driven there, understand the culture, fluent in spanish.

the roads are decent, but sometimes bad. you have to go through check stops and will definitely need good spanish or you will be paying steep bribes. the driving style is completely different.

besides, the bus system is fairly good, extremely cheap, runs often, and will get you where you need to go.

although, if you still want to rent a vehicle, it is definitely possible. Guatemala City has a bunch of major car rental companies.

The main roads in Guatemala are generally very good, however there are security issues and as well your rent a car usually will have a $1,000 plus deductible on your credit card so if damaged or stolen, or you cause an accident, will cost you dearly. In Antigua visit the Rainbow Travel Center, 7a Avenida Sur No. 8, there are Shuttle Bus services available to and from Airport/Guatemala City, Lake Atitialan, ChiChicastenanago, Monterrico Beach and tours to Pacaya and other volcanoes with security, etc. Antigua is very safe during the day and early evening hours, however dark and deserted late at night..take caution and be aware if traveling alone. If you do rent and plan on visiting remote mountainous areas, rent a 4 Wheel Drive with full insurance coverage (expensive but well worth it), teh rainy season starts soon in May. Buen Viaje.
