Very costly. $0.80 to $14 per minute, according to the latest information I saw (on Wikipedia). I did a project on satellite phones back in the 1990s for school, and that was about the price per minute back then, so there is little downward pressure on pricing, apparently.

I don't know a single person in real life that has a satellite phone. They are mostly used by people in remote locations without any means of phone service - workers in developing nations, mining or petrochemical engineers looking for locations to dig, etc. I know that back in the 1990s, many of the services switched over to mobile service when in range. Partly is was due to the costs of the satellite service, but it was also because the mobile phone companies were not happy about manufacturers of phones like Motorola being involved in the satellite services.

We use them for work sometimes.
Being offshore or in an isolated place they are handy to have.
As a traveller, they aren't worth it at the moment but they are getting cheeper (especially with the US dollar going down)
Check out Satellite rates to give you an idea of coverage and pricing.
The phones do cover most of the world, but not it all - different locations different providers.
